Work Permit

All minors must obtain and have filled out the promise of employment form for 14 through 17-year-olds before a work permit can be issued. See below for required forms.

Page 1: Promise of Employment (To be completed by Employer)
Page 2: Physician certificate of health for 14- to 15-year-olds. Requires Parent and Student
signatures.

During the school year, return all completed Work Permit Forms to Mrs. Colby (East) or Ms. Gowans (West) for a permit to be issued.

After June 24, please email the completed paperwork to Rachel Todoroff at: rtodoroff@sturgischarterschool.org. Once the permit is prepared, she will email to arrange a time for the student to sign and receive the permit. Please note that the student must sign the permit in front of the school official.

Students are also able to get Work Permits at their local public school.
