All minors must obtain and have filled out the promise of employment form for 14 through 17-year-olds before a work permit can be issued. See below for required forms.

Page 1: Promise of Employment (To be completed by Employer)
Page 5: Physician certificate of health for 14- to 15-year-olds. Requires Parent and Student
signatures.
Page 6: Sturgis Student Information Form.

Go To Form

During the school year, return all completed Work Permit Forms to Mrs. Colby (East) or Ms. Gowans (West) for a permit to be issued

Students are also able to get Work Permits at their local public school.
