Forms can be found at the bottom of the home page of Parents’ Community Portals. Please note, ALL parents have these forms on the portal, but we only need them to be filled out 1 time per student. The forms are also accessible through the links below and are also on the East and West Campus pages of the website.

The All Student Form is located on the parent/guardian Community Portal and on the East and West campus pages of the website. The Student Handbook is part of that form. The handbook can be viewed on the footer of the Sturgis website and is also available HERE.

All Students need to fill out the Emergency Medical Form.

Google Workspace Terms of Service: As of March 1 2025, Google Workspace for Education requires in its agreement (section 3.5) that schools obtain parent or guardian consent for any Additional Services they allow students under the age of 18 to use. This will impact research assignments in several subject areas, including Theatre, Music and English.
